Treatment of Diabetic Complications in the Retina (Macular Edema and Retinopathy)
What is Retinal Edema?
Retinal edema is one of the most common and serious complications caused by diabetes and other systemic conditions. It occurs when fluid leaks into or between the layers of the retina, particularly affecting the macula (the center of vision). This leakage disrupts the normal function of the retina, leading to blurred or distorted vision, which can progress if left untreated.
Causes of Retinal Edema
Diabetes (Diabetic Retinopathy): The most frequent cause, where high blood sugar damages the small retinal blood vessels.
High Blood Pressure: Weakens blood vessel walls, causing leakage into the retina.
Vascular Disorders: Certain chronic conditions affecting blood vessels can result in retinal fluid accumulation.
Eye Injuries: Trauma to the eye may also trigger retinal swelling.
Symptoms of Retinal Edema
Blurred or distorted vision.
Increasing need for stronger lighting when reading or working.
Appearance of dark spots or wavy lines in the field of vision.
If you experience any of these symptoms, it is crucial to see an eye specialist immediately, as early diagnosis significantly improves treatment outcomes.
Diagnosis and Treatment at Dr. Mohamed Nasr’s Eye Center
At our center, we follow a precise and structured approach to diagnosing and treating retinal edema and diabetic retinopathy:-
Comprehensive Eye Examination: Detecting early signs of leakage or retinal changes.
Advanced Retinal Imaging: Using fluorescein angiography or Optical Coherence Tomography (OCT) to locate and measure the extent of fluid buildup.
Blood Sugar & Blood Pressure Control: To protect the retina.
Intravitreal Injections (Anti-VEGF): To stop abnormal vessel growth and reduce fluid accumulation.
Retinal Laser Therapy (Argon Laser): To seal leaking blood vessels and stabilize the retina.
Laser Photocoagulation: To stop or reduce fluid leakage in specific retinal areas.
Intravitreal Injections & Specialized Procedures: To deliver targeted treatment directly to the retina.
Surgical Intervention (Vitrectomy): In advanced cases, to remove fluid or repair retinal damage.

What is diabetic macular edema (DME) and diabetic retinopathy?
DME occurs when fluid builds up in the macula (center of the retina), impairing vision. Diabetic retinopathy is damage to the retinal blood vessels caused by diabetes.
Can these conditions be cured?
While there’s no cure, treatment can slow progression, preserve sight, and in some cases partially restore vision.
How important is diabetes control?
Excellent blood sugar, blood pressure, and cholesterol control is foundational—slowing the progression of retinopathy and reducing vision damage
What treatments are available for macular edema?
Key treatments include anti-VEGF eye injections (e.g. Eylea, Lucentis, Avastin), laser therapy, and corticosteroids via injection or implant.
How do anti-VEGF injections work and how often are they needed?
These drugs block VEGF to reduce blood vessel leakage. Treatments start monthly, then may be administered every 2–4 months depending on response.
Is there an option with fewer injections?
Yes—a higher-dose version of Eylea (aflibercept) is now FDA-approved and allows for longer intervals between injections, reducing treatment burden.
How does laser treatment help?
Laser photocoagulation seals leaking vessels and reduces swelling; studies show it cuts the risk of moderate vision loss nearly in half.
When is surgery considered?
If there’s significant bleeding or scar tissue, vitrectomy surgery may be needed to remove vitreous hemorrhage and stabilize the retina.
How long does treatment take to work?
Improvements may take months to years. A Cleveland Clinic study notes that resolution of DME can take 1–2 years despite aggressive treatment.
Are there new treatment advances?
Promising research shows that the HIV drug lamivudine (Epivir) improved vision in DME patients, suggesting potential for new oral therapies.
