What is the Keratoconus Procedures?

 Keratoconus is an eye condition that affects the cornea, the transparent front surface of the eye. Instead of maintaining its normal round dome shape, the cornea gradually becomes thinner and bulges outward into a cone shape. This distortion causes light to scatter irregularly inside the eye, leading to blurred and distorted vision, which can make everyday tasks like reading and driving more difficult.

Treatment Options for Keratoconus

The choice of treatment depends on the severity and stage of the condition. At Dr. Mohamed Nasr’s Eye Center, we offer a wide range of advanced procedures for keratoconus, including:

1. Corneal Cross-Linking:-

This procedure is designed to strengthen the corneal fibers and stop the progression of keratoconus by using ultraviolet (UV) light in combination with riboflavin (vitamin B2) eye drops.

  • Epi-off Cross-Linking: The outer layer of the cornea (epithelium) is carefully removed to allow riboflavin to penetrate deeply.

  • Epi-on Cross-Linking: Riboflavin drops are applied without removing the epithelium, making the procedure less invasive and more comfortable for patients.

2. Intracorneal Ring Segments (ICRS) with Femtosecond Laser:-

Using femtosecond laser technology, small ring segments are implanted within the cornea to reshape it. This reduces the cone-like bulging of the cornea and improves vision quality.

3. Laser-Based Treatments:-

In selected cases, laser techniques can be combined with corneal cross-linking to smooth the corneal surface and further enhance visual outcomes.

Corneal collagen cross-linking (CXL) strengthens the cornea using riboflavin and UV-A light. It’s the only intervention proven to stabilize or slow keratoconus progression.

Yes—topography-guided photorefractive keratectomy (PRK) can be combined with CXL to reshape the cornea and enhance visual clarity.

ICRS (e.g. Intacs or Kerarings) are small polymer implants inserted into the cornea’s mid-layer to flatten its shape, improve contact lens fit, and delay the need for a corneal transplant. They’re reversible and minimally invasive.

Yes—combination approaches like Cretan protocol or Athens protocol integrate laser reshaping with cross-linking for enhanced outcomes.

In advanced keratoconus—when scarring, extreme thinning, or instability prevent effective contact lens wear—a corneal transplant (full or partial thickness) may be required.

  • DALK (Deep Anterior Lamellar Keratoplasty) replaces only the front corneal layers—reducing rejection risk and preserving healthy tissue.

  • PK (Penetrating Keratoplasty) replaces the entire cornea and is reserved for more severe cases.

A newer, less invasive surgical option where a donor’s Bowman layer is transplanted to strengthen and flatten very thin corneas, delaying or avoiding transplant.

Yes—recent research explores bio-engineered corneal implants made from pig tissue to treat advanced keratoconus, offering an alternative to human grafts.
